Gathering of Personal Information

Any information and personal data gathered for the purpose of your visit to Vivi Skin Clinic to assist in your treatment, provide quality care and fulfill legislative requirements. Your rights provided in the Health Information Privacy Code 2020 and the Privacy Act 2020 will be respected, including your right to access and, if necessary, correct any information held about you. Collecting Information

We collect personal information from you including:

  • Name

  • Contact information

  • Health information

We collect your personal information in order to provide quality consultation advice and treatments. We only share this information internally and with our Oversight Doctor and the Ministry of Health for reporting and compliance. Providing this information is optional. If you choose not to provide personal contact and health information for example on allergies and medical history, we may not be able to provide treatment services.

We keep your information safe by storing it in encrypted files on a medical application and scheduling application, both are HIPAA Compliant Cloud Technologies. This enables us to secure your information for authorised access only, and to further prevent unauthorised use or disclosure of the information.

We keep your information for five years, at which point we securely destroy it by securely erasing it from our systems.

Use of Cookies

A cookie is a small file that resides on your computer and is recognised by our server when you visit our site. A cookie does not provide any personally identifiable information. It does provide details of your IP address, computer platform, browser, and domain. This information enable us to:
